This Form is meant to keep track of purchases made by our lab. Every person must fill this form out before making a purchase.
  • Instructions

    After asking a Pcard holder to provide payment information for any purchase, you MUST fill this form out immediately upon receiving an invoice from the purchase made. Here are some guidelines you must always follow: 1- Always use the address below for fedex packages: Attention to "your name" 968 center drive Gainesville Fl, 32611 2- Never make Amazon purchases on your own, you must go through Dr. Maghari or the purchasing office. 3- In order to make a payment on an order you must inform Dr. Maghari and the P-Card holder so that they can provide the company rendering service or goods with the billing address, card number, and tax exemption certificate. 4- Fill this form out as soon as you get the invoice for your purchase. The form must be submitted 5 business days after the purchase date, at the latest.
